Announcing Neighborhood Mini-Grants in New Haven!
CARE is awarding $1,500 to each of the six neighborhoods -- including Fair Haven -- that participated in the 2015 New Haven Health Survey to help neighborhoods address health issues they find important in their communities. CARE is seeking proposals for projects that aim to improve health and relate to the major risks for chronic disease by impacting access to healthy foods and healthy eating and increasing physical activity.

CARE conducted health surveys in 2009, 2012, and 2015 in six low-income New Haven neighborhoods: Dixwell, Fair Haven, Hill North, Newhallville, West River/Dwight and West Rock/West Hills.
The 2015 findings reveal that we continue to face many challenges to health and well-being in New Haven's low-income neighborhoods. However, results show some improvements. Please see CARE's report here. We must leverage the power of our community to achieve bigger changes and better health.

Once again Fair Haven Community Health Care has been recognized as a high achieving health center by the U.S. Department of Health and Human Services (HRSA), and has received the gold level designation.
